چکیده

Surgery for portal hypertension began in 1877, with Nicolai Vladirmirovich who performed the fi rst portacaval anastomosis. There was little clinical success until Blakemore and Whipple between the 1920’s and 1940’s: the mortality rate ranged from 25-40%. Hepatic encephalopathy was a clinical problem. In 1960, Warren and collegues performed a distal splenorenal shunt, which reduced the incidence of hepatic encephalopathy to an acceptable level. Other complications after undergoing successful portacaval anastomosis included ammonia toxicity, responsible for the neuropsychiatric changers seen in patients with impending hepatic coma and progressive hypersplenism, inducing thrombocytopenia and leucopenia which can become life-threatening after a shunt procedure. Transjugular intrahepatic portosystemic shunt (TIPS) is a less invasive, non-surgical procedure basis which minimizes complications in the treatment of portal hypertension. The procedure is mostly performed by Interventional Radiologists (IR) and is effective in selected cases of cirrhosis and portal hypertension.

Pulmonary hypertension after transjugular intrahepatic portosystemic shunt (TIPS).

We reported the case of a patient in whom severe, and ultimately fatal, pulmonary hypertension developed 1.5 yrs after transjugular intrahepatic portosystemic shunt (TIPS). Pulmonary artery pressures were not affected by 100% oxygen, prostacyclin or nifedipine. Postmortem examinations showed pulmonary and vascular abnormalities typical of pulmonary hypertension. Pulmonary artery pressures shoul...

Successful Management of Hematuria from Orthotopic Neobladder Varices with Transjugular Intrahepatic Portosystemic Shunt (TIPS) and Variceal Embolization

Hematuria in an orthotopic neobladder is a difficult entity to manage due to its' unique anatomic and physiologic features. Infection, stone disease, primary bowel mass, and recurrent urothelial cell cancer are among the causes to consider. Here, we report a case of a 61 year old male who developed hematuria secondary to neobladder varices which was managed with TIPS (Transjugular Intrahepatic ...

Anatomic Variations of the Right Portal Vein: Prevalence, Imaging Features, and Implications for Successful Transjugular Intrahepatic Portosystemic Shunt Creation

Given the widespread use of transjugular intrahepatic portosystemic shunt (TIPS) creation for the treatment of portal hypertensive complications, a working knowledge of portal venous anatomy is critical for interventional radiologists. The right portal vein - which is most commonly accessed during TIPS - is subject to various anatomic variants that may potentially impact procedure success. This...

Transjugular Intrahepatic Portosystemic Shunt for Refractory Variceal Bleeding in a Patient with a Left Ventricular Assist Device

Transjugular intrahepatic portosystemic shunt (TIPS) is an attractive therapeutic option in cirrhotic patients with recurrent variceal bleeding refractory to medical and endoscopic therapy. There is little known about the use of TIPS in patients with left ventricular assist devices (LVAD). We describe a patient with a durable continuous-flow LVAD and concurrent decompensated cardiac cirrhosis w...

Transjugular intrahepatic portosystemic shunt for chylous ascites in a patient with recurrent cirrhosis following liver transplantation

Chylous ascites (CA) is the extravasation of lipid-rich lymphatic fluid into the peritoneal space following trauma or obstruction of the lymphatic system. Refractory cases of cirrhosis-related CA may be amendable to transjugular intrahepatic portosystemic shunting (TIPS). We present a case of TIPS in the setting of refractory CA secondary to cirrhosis of a transplanted liver graft. Following TI...

The transjugular intrahepatic portosystemic shunt (TIPS).

The creation of an intrahepatic portosystemic shunt via a transjugular approach (TIPS) is an interventional radiological procedure used to treat the complications of portal hypertension. TIPS insertion is principally indicated to prevent or arrest variceal bleeding when medical or endoscopic treatments fail, and in the management refractory ascites. This review discusses the development and exe...
